Kindergarten registration for new students and preschool pre-registration for the 2016-17 school year begins Wednesday, May 18. Packets are available in the elementary office between 8 a.m. and 4 p.m. All registration forms must be filled out and turned in by June 3.
Pre-kindergarten students must be five years of age as of midnight, August 31, 2016.
All immunizations must be completed before the first day of school.
Required immunizations for entry into Kindergarten:
* 5 doses of DTaP or DT vaccine, with the last dose on or after the 4th birthday.
* 4 doses of inactivated polio vaccine (IPV) with the last dose on or after the fourth birthday.
* 2 doses of live virus measles/mumps/rubella (MMR) vaccine on or after the first birthday and at least 28 days apart or laboratory documentation of measles immunity.
* 3 doses of hepatitis B; series takes six months to complete. Series must be started before entry into school.
* 2 doses of varicella (chickenpox) vaccine or date of disease.
* The month/day/year are required on all certificates of immunization forms.
Exemptions (medical, religious/personal)
A child may be released from the obligation of having any or all of the required doses of vaccines under exceptional circumstances. These waivers from participation are called exemptions. They may be granted for medical, personal or religious reasons. Children who have been exempted are susceptible to the disease(s) for which they did not receive vaccine(s). Therefore, in the event of a disease outbreak, they will be excluded from school attendance for the duration of the outbreak. All exemptions must be signed by a physician or other licensed healthcare provider unless it is a religious exemption.
